ABSTRACT

La equinococosis quística es una zoonosis de origen parasitario con distribución cosmopolita. En nuestro país continúa siendo una enfermedad endémica, afectando principalmente pequeños centros poblados y áreas rurales pobres relacionadas a la producción ovina. Los planes de control requieren de un abordaje holístico, con la participación de diferentes instituciones, profesionales y en especial de la comunidad. La cooperación regional busca monitorizar el avance de la enfermedad y centrar las medidas de acción sobre cuatro ejes, definidos según los puntos de intervención más comunes para la quiebra del ciclo de transmisión y la consecuente reducción en incidencia y prevalencia de la enfermedad. La presente revisión utilizó literatura obtenida en diferentes bases de datos, bibliotecas virtuales y sitios web regionales y locales con el objetivo describir las principales estrategias de control, vigilancia y prevención aplicadas actualmente en nuestro país.


Cystic echinococcosis is a zoonosis of parasitic origin with cosmopolitan distribution. In our country it continues to be an endemic disease, affecting mainly small population centers and poor rural areas related to sheep production. Control plans require a holistic approach, with the participation of different institutions, professionals and especially the community. Regional cooperation seeks to monitor the progress of the disease and focus action measures on four axes, defined according to the most common points of intervention to break the transmission cycle and consequently reduce the incidence and prevalence of the disease. This review used literature obtained from different databases, virtual libraries and regional and local websites with the aim of describing the main control, surveillance and prevention strategies currently applied in our country.


A equinococose cística é uma zoonose de origem parasitária com distribuição cosmopolita. Em nosso país, continua a ser uma doença endêmica, afetando principalmente pequenos centros populacionais e áreas rurais pobres relacionadas à produção de ovinos. Os planos de controle exigem uma abordagem holística, com a participação de diferentes instituições, profissionais e, principalmente, da comunidade. A cooperação regional busca monitorar o progresso da doença e concentrar as medidas de ação em quatro eixos, definidos de acordo com os pontos de intervenção mais comuns para interromper o ciclo de transmissão e, consequentemente, reduzir a incidência e a prevalência da doença. Esta revisão utilizou a literatura obtida em diferentes bases de dados, bibliotecas virtuais e sites regionais e locais com o objetivo de descrever as principais estratégias de controle, vigilância e prevenção aplicadas atualmente em nosso país.

ABSTRACT

La hidatidosis pancreática representa el 0,2-0,6 % de los casos, siendo la población pediátrica la de mayor riesgo. Las lesiones suelen localizarse en cabeza del páncreas (50-58 %); la localización en cuerpo y cola del páncreas se encuentra en el 24-34 % y el 19 %, respectivamente. Dada la posibilidad de complicaciones, suele realizarse tratamiento quirúrgico. Se sugiriere indicar albendazol antes y después del acto quirúrgico por los riesgos de ruptura y diseminación de los protoescólices. Se presenta el caso de una niña de 5 años de edad con dolor abdominal progresivo y lesión quística en páncreas compatible con hidatidosis en la ultrasonografía. En la tomografía computada se observa compresión de la vía biliar. La hemoaglutinación indirecta fue negativa. Presentó elevación de la bilirrubina total, con franco predominio de bilirrubina directa, y aumento de enzimas hepáticas. Se realizó laparotomía exploradora, colecistectomía y destechamiento del quiste. Evolucionó favorablemente, continuó con albendazol durante 3 meses luego de la cirugía.


Pancreatic echinococcosis accounts for 0.2­0.6% of cases, with the pediatric population being at a higher risk. Most commonly, pancreatic lesions occur in the head of the pancreas (50­58%); and in the body and tail in 24­34% and 19% of cases, respectively. Given the potential complications, surgery is usually performed. Albendazole is recommended before and after the surgery due to the risks for rupture and dissemination of protoscolices. Here we describe the case of a 5-year-old girl with progressive abdominal pain and cystic lesion in the pancreas compatible with echinococcosis in the ultrasound. The computed tomography showed bile duct compression. Indirect hemagglutination was negative. She had elevated total bilirubin, with a clear predominance of direct bilirubin, and high liver enzymes. Exploratory laparotomy, cholecystectomy, and unroofing of the cyst were performed. The patient had a favorable course and continued with albendazole for 3 months after the surgery.

ABSTRACT

Existen numerosas entidades en la población pediátrica que pueden presentarse en forma de quistes o como lesiones de similares características. De estas patologías, las infecciosas son las más frecuentes. Se presenta el caso de una paciente oriunda de Bolivia con migración reciente a la Argentina que presentó una coinfección con tuberculosis e hidatidosis pulmonar. Ambas infecciones se pueden presentar con signos y síntomas similares y, aunque la asociación citada es poco frecuente en la bibliografía, ciertos mecanismos inmunitarios podrían intervenir en la coinfección de parásitos helmintos y micobacterias. Ambas patologías son infecciones prevalentes en nuestra región y deben ser tenidas en cuenta entre los diagnósticos diferenciales ante pacientes con imágenes quísticas o cavitarias pulmonares.


Numerous entities in the pediatric population can present in the form of cysts or as lesions with similar characteristics. Of the pathologies that can cause these images in children, infectious diseases are the most frequent. We present the case of a native of Bolivia with recent immigration to Argentina who presented a pulmonary co-infection with tuberculosis and hydatidosis. Both infections can present with similar signs and symptoms and although this association is rarely reported in the literature, certain immunological mechanisms could intervene in the causal association of co-infection between helminth parasites and mycobacteria. Both pathologies are very prevalent infections in our region and should be taken into account among the differential diagnoses in patients with cystic or cavitary pulmonary diseases.

ABSTRACT

Nuestro objetivo fue determinar un score que ayude a predecir la probabilidad de infestación canina por Echinococcus en una zona endémica en hidatidosis. Se realizó un estudio longitudinal en 3 anexos de la provincia de Concepción ubicado en los andes centrales de Perú. La infección canina por Echinococcus granulosus fue definido por la presencia de antígeno identificado en la muestra de heces. El modelo predictivo se obtuvo mediante un análisis de regresión logística basado en los parámetros sociodemográficos, esta regla fue internamente validada por remuestreo de tipo bootstrap. Resultados: De 152 canes que se sometieron al estudio de heces, 76 tuvieron infección por Echinococcus confirmada por antígeno reactivo. Se identificaron 4 factores ponderados dentro de la regla de predicción que, en suma, dieron el puntaje: El can que duerme fuera de casa (3 puntos), alimentar con vísceras crudas al can (3 puntos) y sacrificar al ganado dentro de la casa (2 puntos) y el can se alimenta con croquetas (-2 puntos). Se encontró que esta regla de predicción tuvo valor del área bajo la curva ROC 0,78; (IC 95%: 0,70 ­ 0,86). Estos parámetros de predicción con un puntaje ≥ 3 tenía una sensibilidad del 75% y especificidad del 65,8%, con un valor predictivo positivo del 85,3%. Esta regla ayudará al personal de la salud a identificar a los canes infestados por Echinococcus granulosus, para su intervención anti parasitaria y preventiva en sus dueños(AU)


Cystic hydatidosis is an endemic disease in Andean regions, our objective was to determine a score that helps to predict canine Echinococcus infestation in an endemic area with hydatidosis. A longitudinal study was carried out in 3 annexes of the province of Concepción located in the central Andes of Peru. Canine echinococcosis infection was defined by the presence of E. granulosus antigen identified in the stool sample. The predictive model was obtained through a logistic regression analysis based on the sociodemographic parameters, this rule was internally validated by bootstrap type resampling. Results: Of 152 dogs that underwent the stool study, 76 had an Echinococcus infection confirmed by reactive antigen. Four weighted factors were identified within the prediction rule that, in sum, gave the score: The dog that sleeps outside the home (3 points), feeding raw viscera to the dog (3 points), slaughtering the cattle inside the house (2 points) and the dog is fed with croquettes (-2 points). This prediction rule was found to have a value of the area under the ROC curve 0.78 (95% CI: 0.70 - 0.86). At a cutoff point of ≥ 3 points, the prediction rule was found to have a sensitivity of 75% and a specificity of 65.8%, with a positive predictive value of 85.3%. This rule will help health personnel to identify canes infested by Echinococcus granulosus, for their anti-parasitic and preventive intervention in their owners(AU)

Abstract Background and objectives: Hydatid cyst is a zoonotic disease caused by Echinococcus granulosus. The aim of our study is to present the clinical features of the patients who were treated for hydatid cyst, determine the interventional techniques and anesthesia methods used and review the occurred complications in detail. Methods: This study included 393 patients who were followed up and/or treated with the diagnosis of hydatid cyst between January 2013 and November 2018. The patients' data was evaluated retrospectively. Results: The mean age of the patients was 31.0 ± 17.2 years. Of the patients, 111 (28.4%) had more than one cyst and 36 (9.2%) patients had multi-organ involvement. Six of the patients refused the intervention or was transferred to another hospital. Among the remaining 387 patients, 335 (85.2%) received general anesthesia and intubation, 9 patients (2.3%) received general anesthesia and laryngeal mask airway, 39 patients (9.9%) received sedoanalgesia and 4 patients (1%) received regional anesthesia. Perioperative mortality was developed in one patient. The most common periopertaive complication was allergic reaction (1.5%), whereas the most common post-operative complications were atelectasis (3.3%) and biliary fistula (3%). The mean Intensive Care Unit stay (ICU) was 1.9 ± 1.1 days in patients requiring ICU. Recurrence during the 40 ± 17 months follow-up occurred in 8.4% patients. Conclusions: Anesthesiologists have an important role in the management of hydatid cyst patients. Patients should be evaluated exhaustively in terms of multi-organ involvement and the presence of more than one cyst in the same organ. The type of treatment procedure and the localization of the cysts determine the anesthetic management.


Resumo Introdução e objetivos: Cisto hidático é uma zoonose causada por Echinococcus granulosus. O objetivo do estudo é apresentar as características clínicas dos pacientes submetidos a tratamento de cisto hidático, determinar as técnicas intervencionistas e os tipos de anestesia utilizados, e revisar as complicações ocorridas, em detalhe. Método: Este estudo incluiu 393 pacientes que foram acompanhados e/ou tratados com o diagnóstico de cisto hidático, entre janeiro de 2013 e novembro de 2018. Os dados dos pacientes foram avaliados retrospectivamente. Resultados: A idade média dos pacientes foi 31,0 ± 17,2 anos. Do total de pacientes, 111 (28,4%) tinham mais de um cisto, e 36 (9,2%) apresentavam comprometimento em vários órgãos. Seis pacientes recusaram a intervenção ou foram transferidos para outro hospital. Dentre os 387 pacientes remanescentes, 335 (85,2%) receberam anestesia geral e intubação, 9 (2,3%) anestesia geral e máscara laríngea, 39 (9,9%) sedação e analgesia, e 4 (1%) anestesia regional. Houve um óbito no período perioperatório. Reação alérgica foi a complicação perioperatória mais comum (1,5%), e no pós-operatório observou-se mais atelectasia (3,3%) e fístula biliar (3%). O tempo médio de internação na Unidade de Terapia Intensiva foi 1,9 ± 1,1 dias para aqueles que necessitaram desses cuidados. Recidiva durante o seguimento de 40 ± 17 meses ocorreu em 8,4% dos pacientes. Conclusões: Os anestesiologistas têm um papel importante no tratamento dos pacientes com cisto hidático. Os pacientes devem ser avaliados exaustivamente em relação a comprometimento de vários órgãos e presença de mais de um cisto no mesmo órgão. O tipo de procedimento terapêutico e a localização dos cistos determinam a conduta anestésica.

El quiste hidatí­dico es una enfermedad parasitaria humana causada por el estado quí­stico del parasito Echinococcus granulosus. En el Perú hay zonas hiperendémicas de hidatidosis humana. Los quistes hidatí­dicos hepáticos son los más frecuentes, y la ruptura hacia el tórax es rara, siendo menor de 1% de los casos. Presentamos el caso de una paciente con quiste hidatí­dico hepático roto agudamente hacia tórax que es una patologí­a de difí­cil abordaje y de alto riesgo para el paciente y como fue resuelto en forma multidisciplinaria. Hydatid cyst is a human parasitic disease caused by the cystic stage of Echinococcus granulosus. There are hyperendemic areas of hydatidosis in Peru. Liver involvement is the common affection of the disease, rupture of liver cysts into the thorax is rare (less than 1% of cases). We present the case of a female patients who presented with a liver cyst rupture to the thorax, a multidisciplinary approach was needed to cure the patient. (AU)

ABSTRACT

Hydatidosis is a frequent infestation in large endemic areas, caused by helminths. Primary localization within the muscle or bone tissues is rare. We report the case of a 52-year-old woman with a cystic lesion located in the right pectoralis minor muscle, who was initially diagnosed with cystic lymphangioma by imaging examination. She was submitted for surgical treatment; in block resection of the tumor along with the involved muscle was performed. The histopathological diagnosis was of hydatid cyst. The contribution of the ancillary lab tests is analyzed for a precise preoperative diagnostic approach. This case well illustrates that the most likely is not always what it appears to be. Facing of a cystic lesion in the lungs, liver or muscle, clinicians should always think on hydatid disease, particularly in endemic areas.

RESUMEN La hidatidosis es una infección zoonótica que puede invadir múltiples órganos en el ser humano. Sin embargo, el tejido subcutáneo es el órgano menos afectado por esta enfermedad, cuando esto ocurre se aprecia escasa sintomatología, lo que ocasiona periodos prolongados de enfermedad asociado a diagnóstico tardío y a una pobre respuesta a la terapia antiparasitaria. La sierra central del Perú es la zona con mayor prevalencia, presumiblemente por las inadecuadas medidas de saneamiento ambiental, la falta de educación y las casi inexistentes medidas de control de la enfermedad. Con el objetivo de discutir los problemas relacionados a la localización subcutánea y su probable fisiopatología, reportamos dos casos de hidatidosis subcutánea que se presentaron con lesiones tumorales de evolución prolongada y que no afectaron al hígado ni a los pulmones y que finalmente fueron tratados mediante extirpación quirúrgica debido a la falta de respuesta al tratamiento con albendazol.


ABSTRACT Hydatidosis is a zoonotic infection that can invade many organs in the human being. Nevertheless, the subcutaneous tissue is the less affected organ by this disease. When this disease appears, little symptomatology is observed, which causes prolonged periods of disease associated to delayed diagnosis and a poor answer to the antiparasitic therapy. The central mountain range of Peru is the geographical zone with the greatest prevalence, presumably due to the inadequate measures of environmental sanitation, the lack of education and the almost non-existent control measures of the disease. With the aim to discuss the problems related to the subcutaneous location and its probable physiopathology, this study reports two cases of subcutaneous hydatidosis that appeared with tumor lesions with a prolonged evolution and that did not affect the liver or the lungs and which were finally treated by means of surgical removal due to the lack of response to treatment with albendazol.

El diagnóstico presuntivo de la equinococosis quística (EQ) se basa en estudios clínicos e imagenológicos, en particular aquellos que aplican ultrasonido, en datos epidemiológicos, y se confirma con la serología específica. En las zonas endémicas rurales, donde los procedimientos imagenológicos no siempre se encuentran disponibles, al igual que la serología convencional debido a la falta de laboratorios suficientemente equipados, las pruebas de diagnóstico rápido (TDRs) pueden ser una herramienta de gran utilidad. Se evaluó aquí la precisión diagnóstica de VIRAPID® Hidatidosis (Vircell, España), comparado con dos métodos comerciales usados frecuentemente en este laboratorio, para el diagnóstico de la Hidatidosis: HIDATEST (Laboratorio Lemos, Argentina) y ELISA IgG Echinococcus (Vircell, España). El análisis se realizó sobre 224 muestras de igual número de pacientes; 48 (21,4%) pertenecían a pacientes con quistes probablemente hidatídicos, mientras que 10 (4,5%) a pacientes con quistes no parasitarios. Se agregaron a ellos, 166 (74,1%) muestras que resultaron previamente negativas en la casuística de este laboratorio. Para los tres métodos antes mencionados, la concordancia entre los resultados obtenidos fue evaluada por el estadístico Kappa, donde en todos los casos pudo apreciarse un grado de acuerdo casi excelente. La sensibilidad y especificidad de VIRAPID fueron de 94,1%, 98,9%, ELISA 90,6%, 99,4% e HIDATEST 96,0%, 96,2%, respectivamente. Los TDRs para hidatidosis pueden ser útiles en aquellos entornos con escasos recursos para establecer la certeza diagnóstica. La prueba VIRAPID® Hidatidosis demostró tener un muy buen rendimiento, aunque sería interesante evaluar la sensibilidad del test en presencia de quistes inactivos, lo cual, se supone podría plantear problemas para el diagnóstico.


The presumptive diagnosis of cystic echinococcosis (EQ) is based on clinical and imaging studies, particularly those that apply Ultrasound, in epidemiological data, and is confirmed with specific serology. In rural endemic areas, where imaging procedures are not always available, as with conventional serology due to the lack of sufficiently equipped laboratories, rapid diagnostic tests (TDRs) can be a very useful tool. The diagnostic accuracy of VIRAPID® Hydatidosis (Vircell, Spain) was evaluated here, compared with two commercial methods frequently used in the Laboratory, for the diagnosis of Hydatidosis: HIDATEST (Laboratorio Lemos, Argentina) and ELISA IgG Echinococcus (Vircell, Spain). The analysis was performed on 224 samples from the same number of patients; 48 (21.44%) belonged to patients with probable hydatid cysts, while 10 (4.46%) belonged to patients with non-parasitic cysts. A total of 166 (74.10%) samples that were previously negative in the casuistry of our Laboratory were added. For the three methods mentioned above, the agreement between the results obtained was evaluated by the Kappa statistics, where in all the cases an almost excellent degree of agreement could be appreciated. The Sensitivity and Specificity of VIRAPID® was 94.12%, 98.88%, ELISA 90.57%, 99.44% and HIDATEST 96.00%, 96.17%, respectively. TDRs for hydatidosis may be useful in settings with limited resources to establish diagnostic certainty. The VIRAPID® test showed a very good performance, although it would be interesting to evaluate the sensitivity of the test in the presence of inactive cysts, which can pose problems for the diagnosis.


O diagnóstico presuntivo de equinococose cística (EQ) baseia em estudos clínicos e de imagem, em particular aqueles que aplicam ultrasom, em dados epidemiológicos, e é confirmado com a sorologia específica. Em áreas endêmicas rurais, onde os procedimentos de imagens nem sempre estão disponíveis, como acontece com a sorologia convencional devido à falta de laboratórios suficientemente equipamentados, os testes de diagnóstico rápido (TDRs) podem ser uma ferramenta de grande utilidade. A precisão do diagnóstico de VIRapid® hidatidose (Vircell, Espanha) foi avaliada aqui, em comparação com dois métodos comerciais frequentemente usados neste laboratório, para o diagnóstico de Hidatidose: HIDATEST (Laboratório Lemos, Argentina) e ELISA IgG Echinococcus (Vircell, Espanha). A análise foi realizada em 224 amostras do mesmo número de pacientes; 48 (21,4%) pertenciam a pacientes com cistos provavelmente hidatídicos, enquanto 10 (4,5%) pertenciam a pacientes com cistos não parasitários. Adicionaram-se a eles 166 (74,1%) amostras que resultaram previamente negativas na casuística desse Laboratório. Para os três métodos citados acima, a concordância entre os resultados obtidos foi avaliada pela estatística Kappa, onde em todos os casos um grau de concordância quase excelente pôde ser apreciado. Sensibilidade e especificidade de VIRapid® foi de 94,1%, 98,9%, ELISA 90,6%, 99,4% e HIDATEST, 96,0% e 92,6%, respectivamente. Os TDRs para hidatidose podem ser úteis em ambientes com recursos limitados para estabelecer a certeza diagnóstica. O teste VIRAPID® Hidatidose mostrou ter desempenho muito bom, apesar de que seria interessante avaliar a sensibilidade do teste em presença de cistos inativos, o que se supõe pode causar problemas para o diagnóstico.

ABSTRACT

La hidatidosis es una zoonosis producida por el parásito Echinococcus granulosus. En el ciclo zoonótico del parásito, el hombre es un huésped intermediario y sufre la enfermedad tras la ingesta de alimentos y agua contaminados por la materia fecal de animales infectados. En Argentina, la enfermedad constituye un problema importante de salud pública. Suele manifestarse con compromiso hepático y pulmonar. La afectación de otros órganos, que incluyen el corazón, es infrecuente.


Hydatidosis is a parasitic infection caused by the Echinococcus granulosus larvae, transmitted by the ingestion of infected food, characterized by the formation of cysts in vital organs. In Argentina, the disease is spread throughout the territory, constituting an important public health issue. The disease usually affects the liver and lungs. The affection of other organs is rare, and even more uncommon the affection of the heart. We present the case of a disseminated hydatidosis in a pediatric patient, whose initial clinical manifestation was an acute arterial embolism of the right limb caused by the rupture of a hydatidic cardiac cyst.

ABSTRACT

RESUMEN La equinococosis es una infección parasitaria provocada por Echinococcus granulosus, que, en su estado quístico, forma al denominado quiste hidatídico. Presenta morbilidad importante, con posibles secuelas relacionadas con la ubicación, y altos costos debido al tratamiento quirúrgico y farmacológico prolongado. El hígado y el pulmón son las ubicaciones anatómicas más usuales, mucho más raras son el riñón, bazo, cerebro y corazón, este último representa el 0,5 % a 2 % del total de casos. El Perú es un país endémico de esta antropozoonosis y principalmente registra casos procedentes de la sierra central (95 %). Se presenta el caso de una niña de diez años, con diagnóstico de esta entidad, clasificación ecográfica CE 1, grupo clínico 1 (confirmado por anatomía patológica) con posterior tratamiento quirúrgico y farmacológico específico (albendazol). La paciente se recuperó satisfactoriamente de la cirugía practicada, y fue dada de alta a los 16 días, sin complicaciones.


ABSTRACT Echinococcosis is a parasitic infection caused by Echinococcus granulosus, which, in its cystic state, forms the socalled hydatid cyst. It presents important morbidity, with possible sequelae related to the location, and high costs due to surgical and prolonged pharmacological treatment. The liver and the lung are the most common anatomical locations, and much rarer are the kidney, spleen, brain, and heart, where the latter represents 0.5 to 2% of total cases. Peru is an endemic country of this anthropozoonosis and mainly records cases in the central highlands (95%). This paper presents the case of a 10-year-old girl, diagnosed with this disease, CE1 ultrasound classification, clinical group 1 (confirmed by pathological anatomy) with specific surgical and pharmacological treatment (albendazole) afterward. The patient recovered satisfactorily from the surgery and was discharged at 16 days, without complications.

Resumen La equinococosis quística es una zoonosis parasitaria crónica de alta prevalencia en Chile. Se presenta el caso clínico de un varón de 66 años, proveniente de la Región del Maule, con una equinococosis quística músculo-esquelética. Consultó por dolor, aumento de volumen y una fístula en muslo izquierdo, con salida de líquido cristalino. En el estudio imagenológico se identificaron múltiples lesiones quísticas en el ala sacra, hueso ilíaco y tejidos blandos de zona inguinal y muslo izquierdo. La serología Elisa IgG para Equinococcus granulosus fue positiva. Se realizó la resección quirúrgica de las lesiones musculares y se inició terapia antiparasitaria combinada con albendazol y praziquantel, con buena respuesta clínica; sin embargo, al suspender la terapia, por iniciativa del paciente, se reiniciaron los síntomas.


Cystic echinococcosis is a chronic parasitic zoonosis of high prevalence in Chile. We report a clinical case of a 66-year-old man, domiciled in an urban area of the Maule Region, who presents skeletal muscle cystic echinococcosis. Consultation for pain, volume increase and left thigh fistula that gives out crystalline fluid. In the study with imaging techniques, multiple cystic lesions are identified in the sacral wing, iliac bone, soft tissues of the groin and left thigh. No cysts were evident in other organs. Serology Elisa IgG was positive Echinococcus granulosus. Surgical resection of soft tissue injuries. Combined antiparasitic therapy with albendazole and praziquantel was started, with good clinical response. Upon discontinuation of antiparasitic therapy at the initiative of the patient, symptoms are reinitiated.

In urban populations of South America, dogs with free access to public areas represent a public health concern. The primary consequence of roaming dogs on human health is the transmission of infectious and parasitic diseases mainly through feces contamination. The main diseases likely to be transmitted are hydatidosis or echinococcosis, larva migrans, and giardiasis. In Argentina, hydatidosis ranks among the most prevalent zoonosis. Although it is considered a rural disease, the circulation of this parasite in urban areas has been documented. The aim of this work was to survey intestinal parasites in canine feces from two low-income urban neighborhoods of Bariloche city, Argentina, and to assess their seasonal variation. During 2016, 188 fresh dog feces were collected from sidewalks in 40 randomly selected blocks from the neighborhoods. Each sample was processed by Sheater flotation and tested for a coproantigen (CAg) by ELISA. The percentage of parasitized feces was 65.3% (95% CI: 55.9%-73.8%). Eleven parasite species were found, 3 protozoan, 3 cestodes, and 5 nematodes. Echinococcus sp. was present in 9.3% of the samples (95% CI: 4.7%-16.1%). Canine echinococcosis rates resulted similar to rates found previously in other neighborhoods of the city. The life cycle of Echinococcus sp. is sustained in urban areas by the entry of parasitized livestock, domiciliary slaughtering, and inadequate deposition of offal. The risk of Echinococcus sp. transmission to people in these neighborhoods is very high, due to high density of free-roaming dogs and high percentages of infected feces, similar to percentages observed in rural areas.


En las poblaciones urbanas de América del Sur, los perros con acceso libre a áreas públicas representan un problema de salud pública. La principal consecuencia es la transmisión de enfermedades infecciosas y parasitarias a través de la contaminación por heces. Las principales enfermedades que pueden transmitirse son hidatidosis, larva migrans y giardiasis. En Argentina, la hidatidosis es una de las zoonosis más prevalentes y aunque es considerada una enfermedad rural, algunos estudios muestran la circulación de este parásito en zonas urbanas. El objetivo fue registrar los parásitos intestinales en heces caninas de dos barrios de bajos ingresos de la ciudad de Bariloche, Argentina, y evaluar su variación estacional. Durante 2016, se recolectaron 188 heces frescas de perros en 40 manzanas seleccionados aleatoriamente. Las heces se procesaron mediante flotación de Sheater y una prueba ELISA de coproantigeno (CAg). El porcentaje de heces parasitadas fue del 65.3% (IC 95%: 55.9%-73.8%). Se encontraron 11 especies de parásitos, 3 protozoos, 3 cestodes y 5 nematodes. Echinococcus sp. estuvo presente en el 9.3% de las heces (IC 95%: 4.7% -16.1%). La equinococosis canina mostró valores similares a estudios previos en otros barrios de la ciudad. El ciclo de vida Echinococcus sp. se mantiene en las zonas urbanas por entrada de ganado parasitado, faena domiciliaria y deposición inadecuada de vísceras. El riesgo de transmisión de Echinococcus sp. en estos barrios es alto, debido a la alta densidad de perros sueltos y al alto porcentaje de heces infectadas, similar al de las zonas rurales.

Background: Hydatid disease is a serious parasitic disease threatening public health. Because of its rarity in non-endemic coastal areas, determining the nature and origin of a chronic, enlarged liver cystic mass is challenging in these regions. Under these circumstances, physicians need a confirmatory diagnostic tool beyond immunological and radiological examinations. This study investigated a novel human single-chain fragment variable (scFv) antibody for the confirmative diagnosis of 18 atypical hydatid disease cases in non-endemic coastal areas. Results: A scFv antibody against cystic echinococcosis was produced by genetic engineering and then applied to the immunohistochemical diagnosis of 18 cases of cystic echinococcosis presented in non-endemic coastal areas. The diagnosis of these cases by ultrasound and serum-based examinations was inconclusive. The 750 bp scFv antibody gene was expressed in COS-7 cells, and the antibody localized in the cytoplasm. The scFv antibody can detect the germinal layer and protoscolices of actively growing cysts but not of the degenerating protoscolices and has a diagnostic efficiency higher than that of single serum or ultrasound testing (P b 0.05). The combined use of scFv antibodies with serology and ultrasound diagnostics results in a diagnostic efficiency comparable to that of surgery. The scFv antibody can be used as a confirmatory test for the diagnosis of hydatid disease in non-endemic areas, providing a beneficial supplementary diagnostic method that complements traditional immune testing and ultrasonic radiology and thus helping physicians to effectively differentiate hydatid disease.

RESUMEN El objetivo del presente estudio fue determinar la pérdida económica que ocasiona el tratamiento de la hidatidosis humana, para lo cual se realizó un análisis de costos de los casos reportados en la región Junín en Perú, durante el año 2013. El costo total (directo e indirecto) en dólares americanos por paciente atendido por el Ministerio de Salud (Minsa) fue USD 1 220,0; por el seguro social (EsSalud) fue USD 3 051,0 y por la atención privada fue USD 4 090,7. La pérdida media anual según el número de atenciones por hidatidosis humana no complicada fue USD 24 542,0 en el Minsa, USD 369 292,0 en EsSalud y USD 130 566,4 en la atención privada. Las pérdidas de productividad humana constituyen la minoría. Nuestros hallazgos indican que la hidatidosis impone una pesada carga económica en la región Junín.


ABSTRACT The objective of the present study was to calculate the economic loss caused by the treatment of human hydatidosis. For this purpose, a cost analysis of the cases reported in the Junin region of Peru in 2013 was conducted. The total cost (direct and indirect) per patient assisted by the Ministry of Health (Ministerio de Salud-Minsa), the social security system (EsSalud), and private health care was 1,220, 3,051, and 4,090 US dollars (USD), respectively. The mean annual loss considering the number of health care services for uncomplicated cases of human hydatidosis was USD 24,542 in Minsa, USD 369,292 in EsSalud, and USD 130,566 in private health care. Overall, the rate of loss of human productivity was a small proportion. Our findings indicate that hydatidosis imposes a heavy economic burden in the Junin region of Peru.

RESUMEN Los tumores quísticos primarios de la órbita, causados por infestaciones parasitarias como el Equinococcus granulosus son raros. Al ser el Perú zona endémica para este parásito, presentamos el caso de una niña de 10 años de edad, proveniente de una zona rural de Huancavelica, con proptosis axial irreductible del globo ocular derecho, de curso progresivo, con pérdida de la agudeza visual e imágenes tomográficas de la órbita en relación a masa tumoral de apariencia quística. Los exámenes serológicos fueron negativos, la paciente fue sometida a resección quirúrgica con resultado histopatológico de membranas anhistas correspondientes a quiste hidatídico. Al año de seguimiento no se evidencia recurrencia local ni sistémica. En conclusión, ante una tumoración orbitaria quística en pacientes pediátricos, se debe de considerar como diagnóstico diferencial la hidatidosis orbitaria, sobre todo si proceden de zonas endémicas.


ABSTRACT Primary orbital cystic tumors caused by Echinococcus granulosus are rare. As this parasite is endemic in Peru, we report the case of a 10-year-old girl from a rural area of Huancavelica, who presented with progressive and irreducible axial proptosis of the right eye and loss of vision. Orbital tomographic images demonstrated a tumor mass with a cystic appearance. Serologic tests were negative. The patient underwent surgical resection, and histopathologic analysis revealed anhistic membranes, corresponding to hydatid cysts. During one year of follow-up, no evidence of local or systemic recurrence was observed. In conclusion, orbital hydatidosis should be considered in the differential diagnosis of a cystic orbital tumor in pediatric patients, especially if the patient is from an endemic area.

ABSTRACT

La hidatidosis es una zoonosis con una amplia distribución en Argentina y en el mundo. Se adquiere en la infancia, pero se presenta, más frecuentemente, en la edad adulta. La población pediátrica representa el 10%-20% del total de los casos. Se presentan 47 niños con diagnóstico de hidatidosis durante un período de 20 años. La mediana de edad fue de 8 años (rango: 3-17) y, en su mayoría, tenían algún antecedente epidemiológico orientador, como la procedencia de un área endémica, rural y/o la tenencia de perros alimentados con visceras. El 85% fueron quistes únicos localizados en un solo parénquima. Las localizaciones quísticas pulmonares y hepáticas fueron las más frecuentemente encontradas. El tratamiento fue médico-quirúrgico en 45 pacientes (96%). Trece (28%) presentaron alguna complicación posterior, que fue más frecuente en aquellos con localización pulmonar. Tres (6%) recayeron entre el mes y los 24 meses, y requirieron una nueva cirugía. Ninguno falleció a causa de la enfermedad parasitaria.


Echinococcosis is a zoonotic disease that is widely spread across Argentina and worldwide. It is acquired during childhood but it is more common during adulthood. The pediatric population accounts for 10-20% of all cases. This study included 47 children diagnosed with echinococcosis over a 20-year period. Their median age was 8 years old (range: 3-17); most patients had some epidemiological history, such as having lived or living in an endemic or rural area and/or having dogs that are fed with viscera. Findings included 85% of single cysts in only one parenchyma. Cysts were most commonly located in the liver and lungs. Medical/surgical treatment was carried out in 45 patients (96%). Subsequent complications were observed in 13 patients (28%), which were more common in those with lung cysts. Three patients (6%) relapsed 1-24 months later and required a new surgery. No patient died as a result of the parasitic disease.
